Just click the pink button below - if you're not already logged in to this website, you'll be asked to sign in or create a Minster FM user account (don't worry, it's free and easy). We'll then ask a few quick questions about you and your listening habits.

Once you're signed up, we'll let you know by email when there's a survey available.

Only one entry per individual will be counted.
One person who has completed each survey will be randomly selected as the winner.
There are no cash alternatives, refunds or exchanges available.
The prize is non-transferable, non-negotiable and cannot be substituted for any other prize or cash value, nor can it be used in conjunction with any other offer.
This prize is subject to availability.
This prize may be withdrawn at any time.

Winner will notified by email or phone shortly after the survey ends.
